In detail, all codes for SMS to 13033 for circulation.
In addition a document for working situations (type A form), for different movements with codes 1-6 (type B form), when selecting:
a) text message
b) using a printed completed form downloaded
Or
c) a handwritten statement.
1. Sending SMS
Messages must be sent by phone to 13033
The SMS to 13033 must be in the form of:

x which is your code number, 1, 2, 3, 4, 5, 6
- Routes to school or visit to the doctor, required by the relevant communication. VALID AFTER 21:00
- Routes for the immediate supplies of first-use goods (supermarkets kits, mini markets), or whatever cn not be delivered home. NOT VALID AFTER 21:00
- Travelling to a bank as long as you can not use on line banking. NOT VALID AFTER 21:00
- Travelling to help someone in need who are unable to take care of themselves. NOT VALID AFTER 21:00
- Travelling to a funeral ceremony under the requirements by law or for divorced parents according to the law to attend to their children’s needs. NOT VALID AFTER 21:00
- -Personal exercise, adhering to the restrictions ans social distancing at all times (1.5 meters). NOT VALID AFTER 21:00. Also-Movement with a pet, individually or up to two people only, adhering to the restrictions and social distancing of (1.5 meters) . All excerises should be in close to your home. VALID AFTER 21:00
You will receive in response immediately:

The service is available from mobile phones with SIM card of mobile telephony providers operating in Greece (Cosmote, Vodafone GR, Wind). In order to use the 13033 service, citizens with a device that has a SIM network card abroad, can obtain a prepaid connection from the points of sale of the providers. It is reminded that the use of the 13033 service is free.
The messages of 13033 relate exclusively to confirmation of exceptional movement. 13033 does not send messages referring to software installation or to a website other than forma.gov.gr

3. Handwritten Certificate
The handwritten certificate must have the following information:
First name – Last name
Address
Reason for movement that falls into the above permitted categories and destination address
Date, time, signature
