Identification of insured persons upon their admission to a nursing home,
such as a public hospital, a private clinic, a KAA, a MXA, with protocol number

states the following:
Please be advised that the printed health books of insured persons, pensioners and their family members have been abolished in accordance with article 67 of Law 4603/2019 (Government Newspaper 48 / Α / 14-3-2019).
Identification of insured persons upon their admission to a hospital, such as a public hospital, a private clinic, a KAA, a MXA, will be certified by any legally valid public document (police ID, passport).
Elimination of health booklets forms (sickness).
With the 2022/49 6.3.2019 amendment of the Ministry of Labour, Social Security and Social Solidarity on the regulation of issues of the Ministry of Labour, Social Security and Social Solidarity – on Analytical Periodic Declarations (APD), which has been filed in the bill of the Ministry of Culture and Sport entitled “Professional Sports Committee and other provisions”, the printed health book (sickness) is abolished, since an electronic system for the prescribing and registration of health data has been established in the Personal Electronic Health File.

Article 67
Abolition of Health Books
1. Printed health record booklets of insured persons, pensioners and their family members are abolished.
2. a. Section d of paragraph 1 of Article 2 of the Presidential Decree. 121/2008 (Α 183), as replaced by paragraph 1 of Article 96 of Law 4172/2013 (Α 167), is replaced by the following:
“d”. To verify and to ascertain whether the person present for examination is identical to what appears or is depicted in any public document from which the identification results’.
b. In the case f of par. 1 of article 2 of the Presidential Decree. 121/2008 the words “from the health booklet” are deleted.
3. The twelfth paragraph of paragraph 2 of Article 4 of the Presidential Decree. 121/2008, as replaced with paragraph 6 of Article 96 of Law 4172/2013 and the last sentence of paragraph 2 of article 4 of the Presidential Decree. 121/2008, as replaced by paragraph 8 of Article 95 of Law 4172/2013, are abolished.
4. Any other general or specific provision shall be abolished.
